Do you or a family member need assistance managing challenges and transitions that occur with aging? JFS care managers are aging experts, committed to providing excellent service delivery and care coordination that maintains the dignity and achieves the goals of older adults served. Service options are individualized and could be a one-time discussion/consultation or regular, ongoing support. 

JFS care managers: 

  • Conduct in-home assessments of each person's strengths and needs 

  • Develop a comprehensive, individualized care plan designed to address those needs 

  • Maintain regular contact with family members, near or far, when appropriate 

  • Coordinate and monitor in-home services 

  • Provide healthcare advocacy 

  • Assist with long-term care planning 

  • Give information, referrals, and assistance connecting to additional community resources 

The Aging Care & Connections team helps older adults maintain as much independence as possible, while preparing for unknowns that may occur with age. They work alongside our clients to develop a plan of care that ensures opportunities for self-sufficiency, support, and dignity. They make connections to local and national resources for home care, healthcare, mental health support, nursing, transportation, and safety as needed.
