Family Safety Net (FSN) Rapid Re-Housing Navigator – 26 hours/week

This position provides appropriate housing stability and support services for homeless participants referred by the Denver MDHI OneHome system. Provides services following Housing First, Trauma Informed Care, and Progressive Engagement principles. Job duties include identifying housing for program participants, recruiting, engaging, developing, and maintaining landlord relationships.

Qualifications:

  1. Education: Bachelor degree in social work or closely related human service degree, or comparable navigation services experience.
  2. Experience: Requires 2-4 years’ experience in the social work or human service field. 
  3. Other: Demonstrates strong communication, organizational and interpersonal skills. Knowledge of landlord and tenant rights. Understands Rapid Re-Housing principles. Comfortable working with landlords and advocating for program participants. Knowledge of the service population’s cultural and socioeconomic characteristics and appropriate techniques and practices for working with the client population. Ability to interact effectively as a team member and independently with agency staff, community members, and with a diverse client base. Ability to multi-task in a fast-paced environment. Must have excellent computer skills.

Responsibilities:

Housing Navigation

  • Locate affordable and appropriate and desirable housing for participants.
  • Work with the OneHome system for participant referrals.
  • Engage and recruit landlords and maintain good landlord relationships.
  • Verify literal homeless status, following HUD guidelines.
  • Conduct housing habitability inspections.
  • Assist with leasing appointments.
  • Mediate with landlords and program participants.
  • Help clients maintain stable housing.

Housing Stabilization Case Management

  • Assist case management team to stabilize client housing.
  • Assess client housing barriers and help alleviate them.
  • Input client information into databases as required, including HMIS
  • Authorize services of the program based on assessment of need, grant criteria, and availability of resources.
  • Maintain accurate case records, document client contacts and progress. 
  • Participate in staff training as required.
  • Complete or provide input to reports documenting program activities.
  • Adhere to all grant eligibility and documentation requirements
  • Provide connections to community resources.
  • Understand and adhere to Rapid Re-Housing rules and MDHI Written Standards for RRH
