Boulder JFS Counselor/Care Manager II

Will provide care management and counseling services to older adults, adults with disabilities, individuals experiencing poverty and impacted family members in the Boulder Jewish Family Service (JFS) program.

Qualifications:

Master’s degree in Social Work, Psychology, Gerontology, or related human services field required. LCSW and established Medicare provider preferred.     

Experience: 

Minimum of two years’ experience working with older adults, adults with chronic illness/disability, and/or individuals experiencing poverty, including client interviewing, assessment, care coordination and counseling.

Additional Attributes:

  • Effective interpersonal communication skills and comfortable conducting face-to-face assessments in clients’ homes.
  • Knowledge of resources and available interventions.
  • Understanding of Jewish community and traditions, preferred.
  • Strong technology skills; experience with electronic record keeping and data base systems.
  • Must be organized; attention to detail.Able to work autonomously.

Responsibilities:

  • Completes assessments to identify client service and support needs.
  • Establish care plan goals with client as part of a comprehensive plan of care.
  • Provide therapy to individuals – older and younger adults.
  • Provides ongoing service coordination thru evaluation of services provided.
  • May provide direct assistance with transportation to appointments.
  • Complete in-home or in-office consultations with clients and/or family members.
